Position Overview:
Job Title: In-Home Respite Counselor (DSP)
Location: Rochester, NY (community-based; primarily in participants' homes)
Department: Community Services - In-Home Respite Program
Reports To: Senior Coordinator or Self-Directed Designee
Employment Type: Per Diem / As Needed
Starting Wage: $17.19 per hour

Your Core Responsibilities:
- Provide respite services to families and individuals in the In-Home Respite Program, offering relief to caregivers and companionship to individuals with disabilities.
- Deliver all assigned service hours in the person's home or in community settings (social activities, appointments, stores, etc.).
- Provide face-to-face support and document only face-to-face time for billing purposes.
- Assist with personal care tasks such as toileting, dressing, and eating as needed.
- Work independently while maintaining communication with your supervisor regarding schedules, absences, or changes.
- Complete all required documentation accurately and submit by assigned deadlines.
- Ensure the safety, oversight, and protection of assigned individuals through ongoing risk assessment and monitoring.
- Follow all OPWDD and NYS Justice Center reporting requirements.
- Adhere to Lifetime Assistance's mileage, transportation, and service authorization policies.
- Attend staff meetings and training as requested.
What You Bring:
- High school diploma or equivalent preferred.
- Must hold a valid NYS Driver's License (have had your drivers license for 6 months), have a reliable vehicle, and meet agency vehicle operator requirements.
- Ability to read and interpret service plans, safety procedures, and related documentation.
- Strong written and verbal communication skills to interact with individuals, families, and service providers.
- Ability to solve practical problems, think independently, and adapt to different environments.
- Physical ability to assist with personal care tasks and lift up to 35 pounds as needed.
