Compensation

• Earn $50–$54 per hour based on actual visit time

What You'll Do

• Complete skilled nursing visits in patients' homes

• Perform patient assessments and ongoing nursing evaluations

• Administer medications and provide medication management

• Perform wound care and other skilled nursing treatments

• Educate patients and family members on diagnoses, medications, and home care

• Communicate changes in patient condition to physicians and supervising clinicians

• Complete accurate electronic documentation following each visit

Qualifications

• Current New Hampshire Licensed Practical Nurse (LPN) license required

• Valid driver's license and reliable transportation

• Home care, hospice, rehabilitation, skilled nursing, or long-term care experience preferred

• Strong assessment, communication, and documentation skills

• Ability to work independently and manage your daily schedule

Coverage Area

Visits are available throughout Milford, Amherst, Wilton, Hollis, Brookline, Merrimack, Nashua, Hudson, Peterborough, Greenville, Mason, and surrounding Southern New Hampshire communities. Assignments are scheduled based on your availability and preferred travel radius.
