Our leading Home Health Agency is seeking a compassionate per diem Occupational Therapist for home visits in Norwalk and surrounding areas.

Choose your own hours and make a difference in patients’ lives!

We offer offering skilled nursing, rehabilitation and non-skilled services for the residents in the community that require support with ADL's that may have a physical and/ or developmental disability.

As a Occupational Therapiston our team, you will assess patients, create treatment plans, provide exercises, track progress, and educate on injury prevention while collaborating with the healthcare team.

Qualifications:

  • Licensed Occupational Therapist in Connecticut

  • Reliable transportation, and a willingness to cover Norwalk and surrounding areas

  • Proven experience in Occupational therapy, preferably in a home health or clinical setting

  • Strong understanding of musculoskeletal and neurological conditions

  • Excellent communication and interpersonal skills

  • Ability to develop and implement individualized treatment plans

  • Knowledge of adaptive equipment and assistive devices

Responsibilities:

  • Evaluate patients' physical conditions and create personalized treatment plans

  • Develop and implement therapeutic exercises to improve mobility, strength, and function

  • Monitor patient progress and adjust treatment plans as needed

  • Educate patients and their families on exercises, safety techniques, and home care

  • Collaborate with healthcare teams to ensure comprehensive care

  • Maintain accurate and up-to-date patient records

  • Provide guidance on injury prevention and rehabilitation strategies
