Care Central Professionals Home Health is seeking a dedicated Occupational Therapist (OT) to join our compassionate and patient-focused team in Roseville, CA. Conduct skilled OT evaluations and develop individualized care plans.
Provide interventions to improve safety, independence, and quality of life.
Educate patients, caregivers, and families on adaptive strategies, equipment use, and home safety.
Collaborate with our interdisciplinary team to ensure holistic, patient-centered care.
Maintain accurate, timely, and compliant documentation.
Current California OT license in good standing.
Graduate of an accredited Occupational Therapy program.
At least 1 year of clinical experience (home health preferred).
Excellent communication and organizational skills.
Valid driver’s license and reliable transportation.

Current California OT license in good standing.

Minimum of 1 year of clinical experience (home health experience preferred).
Strong communication and documentation skills.
Ability to work independently and manage time effectively.
Valid driver’s license and reliable transportation.
