What You’ll Do
- Perform PT evaluations and follow-up visits per physician orders
- Develop and adjust individualized plans of care
- Deliver therapy for gait, balance, mobility, strength, and fall prevention
- Train patients and caregivers on home exercises and safe mobility strategies
- Document per NJ DOH, Medicare, and BrightStar standards
- Collaborate with RNs, OTs, STs, and CHHAs
What We Need
- NJ Physical Therapy license in good standing
- 1+ year of PT experience (home health preferred)
- CPR/BLS certification
- Reliable transportation within Camden/Cherry Hill
- Strong documentation and patient communication skills
