We are seeking a Pharmacist to join a hospital-based healthcare team providing care to patients with complex medical needs. This role offers a highly collaborative environment where pharmacists work closely with physicians, nurses, patients, and families to support medication management and optimize patient outcomes.

Schedule

• PRN - Day shifts

Key Responsibilities

• Perform medication reconciliation and assist with discharge medication planning.
• Provide medication education and counseling to patients and caregivers.
• Collaborate with the interdisciplinary care team regarding medication therapy recommendations.
• Participate in antimicrobial stewardship activities, including review of culture results and therapeutic recommendations.
• Monitor and manage pharmacokinetic dosing and anticoagulation therapy.
• Evaluate medication regimens for appropriateness, interactions, adverse effects, and therapeutic effectiveness.
• Contribute to medication safety, quality improvement, and regulatory compliance initiatives.

Preferred Qualifications

• Hospital or inpatient pharmacy experience preferred.
• Experience with antimicrobial stewardship programs.
• Pharmacokinetic dosing and anticoagulation management experience preferred.
• Electronic medical record experience preferred.
• Strong communication skills with the ability to work effectively with patients, families, and healthcare professionals.
• Comfortable caring for patients with a broad range of medical conditions and medication therapies.
