We are seeking a Pharmacist to join a hospital-based healthcare team providing care to patients with complex medical needs. This role offers a highly collaborative environment where pharmacists work closely with physicians, nurses, patients, and families to support medication management and optimize patient outcomes.
Schedule
PRN - Day shifts
Key Responsibilities
Perform medication reconciliation and assist with discharge medication planning.
Provide medication education and counseling to patients and caregivers.
Collaborate with the interdisciplinary care team regarding medication therapy recommendations.
Participate in antimicrobial stewardship activities, including review of culture results and therapeutic recommendations.
Monitor and manage pharmacokinetic dosing and anticoagulation therapy.
Evaluate medication regimens for appropriateness, interactions, adverse effects, and therapeutic effectiveness.
Contribute to medication safety, quality improvement, and regulatory compliance initiatives.
Preferred Qualifications
Hospital or inpatient pharmacy experience preferred.
Experience with antimicrobial stewardship programs.
Pharmacokinetic dosing and anticoagulation management experience preferred.
Electronic medical record experience preferred.
Strong communication skills with the ability to work effectively with patients, families, and healthcare professionals.
Comfortable caring for patients with a broad range of medical conditions and medication therapies.
