Home Health Physical Therapist (PT) for neuro patients
Location: Yucca Valley, CA (2 hours/week) and Soledad, CA (2 hours/week)
PT Pay Rate: $60-$67
Schedule: Part-Time | 4 Hours Per Week Total
Soledad needs: ASAP - September 26, 2026
Yucca Valley needs: ASAP - 8 weeks
Requirements
Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy from an accredited program.
Active and unrestricted California Physical Therapist license.
Experience working with traumatic brain injury (TBI) patients, neurological rehabilitation, or home health preferred.
Valid driver’s license and reliable transportation.
Ability to work independently and provide compassionate, patient-centered care.
Position Overview
We are seeking a Physical Therapist to provide home health services to patients recovering from traumatic brain injuries and other neurological conditions. This role focuses on helping patients improve mobility, strength, balance, and independence within their home environment.
Responsibilities
Conduct patient evaluations and develop individualized treatment plans.
Provide skilled physical therapy services in the patient’s home.
Onboarding documents:
Required Documents:
1. Copy of therapy licenses in California (and any other states you may be licensed in)
2. Copy of driver’s license (including license number)
3. Liability insurance
4. NPI number
5. Resume
Monitor patient progress and modify treatment plans as needed.
Collaborate with caregivers and other healthcare professionals involved in the patient’s care.
Maintain timely and accurate clinical documentation.
