Per visit rate $75.00

We provide skilled nursing, rehabilitation, and non-skilled services to community members needing support with ADLs and communication challenges related to physical, developmental, or cognitive conditions.

As aSpeech Language Therapist, you will evaluate communication and swallowing needs, develop treatment plans, provide therapeutic interventions, track patient progress, and educate families—while collaborating closely with the interdisciplinary care team.

Qualifications

  • LicensedSpeech Language Pathologist (SLP)in Connecticut

  • Reliable transportation and willingness to cover Meriden and surrounding areas

  • Experience as an SLP—home health, outpatient, school, or clinical setting preferred

  • Strong knowledge of communication, speech, language, cognitive-communication, and swallowing disorders

  • Excellent communication, documentation, and interpersonal skills

  • Ability to develop and implement individualized treatment plans

  • Familiarity with AAC devices, adaptive communication tools, and patient/family education

Responsibilities

  • Conduct comprehensive speech, language, cognitive-communication, and/or swallowing evaluations

  • Develop and implement evidence-based, individualized treatment plans

  • Provide therapeutic interventions to address communication and swallowing needs

  • Monitor progress and update goals and care plans as needed

  • Educate patients, families, and caregivers on communication strategies and home programs

  • Collaborate with nurses, therapists, coordinators, and physicians to support patient outcomes

  • Maintain accurate, timely patient records and documentation
