Per Diem Speech Therapist for Home Health Agency - Stuart and Surrounding Area

Job Description

As a Per Diem Speech Therapist, you will provide evaluation, assessment, and treatment services to patients with speech, language, and swallowing disorders. You will work collaboratively with other healthcare professionals to develop and implement individualized treatment plans that meet the unique needs of each patient. This position offers flexibility and the opportunity to work with a diverse patient population in a home health setting.

Key Responsibilities: 

• Conduct thorough assessments and evaluations of patients to determine their speech, language and swallowing abilities
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide therapy services to patients in their homes, utilizing evidence-based practices
• Educate patients and their families on speech and language disorders, treatment plans as necessary
• Maintain accurate and timely documentation of patient evaluations, treatment plans and progress notes in compliance with regulatory standards
• Collaborate with other healthcare professionals, including physicians, nurses and occupational therapists to ensure comprehensive patient care
• Participate in ongoing professional development and training to stay current with industry trends and best practices

Qualifications: 

• Master’s degree in Speech-Language Pathology from an accredited program
• Valid state license or certificate to practice as a Speech Therapist
• Previous experience in a home health setting is preferred but not required
• Strong communication and interpersonal skills to effectively interact with patients, families and team members
• Ability to work independently and manage a flexible schedule
• Knowledge of Medicare and other third-party payor regulations related to home health services is a plus
