    Join Chapa-De Indian Health as a Perinatal Health Representative and become an important part of our Maternal Child Health team. In this role, you’ll work directly with patients throughout pregnancy, postpartum, newborn, and pediatric care, helping coordinate services, provide education, navigate resources, and remove barriers to care.

    This is a great opportunity for someone with experience in maternal child health, women’s health, pediatrics, medical assisting, or patient-care support who enjoys building relationships with patients and helping families access the care and resources they need.

    Hourly Pay: $27.35 - $28.00

    Schedule: Monday- Friday 8:00 AM - 5:00 PM

    What You'll Do:

    · Support patients throughout pregnancy and postpartum care, including assessments, education, care plans, follow-up, outreach, and resource navigation

    . Coordinate prenatal, postpartum, newborn, pediatric, and group-visit services, including scheduling, eligibility verification, patient outreach, and care-gap follow-up

    . Assist patients with Medi-Cal applications, Presumptive Eligibility, insurance eligibility, prior authorizations, pregnancy-related forms, and other resources

    . Track important laboratory, imaging, genetic screening, ultrasound, immunization, and patient-care needs to support timely completion of services

    . Provide and coordinate patient education and childbirth education, while collaborating with case managers, providers, and community partners

    What You'll Bring:

    · 1+ year of experience in maternal child health, women’s health, pediatrics, medical assisting, or comparable patient-care support

    . 1+ year of electronic health record (EHR) experience and current Basic Life Support (BLS) certification

    . High school diploma or GED, plus proficiency in Microsoft Word and Excel, including the ability to use and update formulas

    . Strong organization, communication, prioritization, and ability to work independently while managing multiple patient needs and deadlines

    . Sensitivity to the health and cultural needs of Native American and vulnerable populations
