Responsibilities

• Provide occupational therapy assessments and develop individualized treatment plans.

• Deliver on-site OT interventions to improve daily living skills and functional independence.

• Monitor patient progress and adjust goals based on measurable outcomes.

• Document assessments, treatment plans, and outcomes in compliant electronic health records.

• Collaborate with interdisciplinary team to coordinate care and ensure seamless services.

• Utilize data and pipelines to forecast patient needs and service demand.

• Meet performance targets and contribute to territory/service line growth.

• Educate patients and families on therapy goals, home programs, and safety.

• Identify opportunities for professional development and pursue relevant certifications.

• Participate in quality improvement initiatives to enhance patient outcomes.

• Represent the OT program in team meetings and community events to raise visibility.

Experience/Qualifications

• OT licensure eligible in applicable state with active license

• Licensed Occupational Therapist with 2+ years clinical OT experience

• Demonstrated ability to develop and implement individualized treatment plans

• Ability to meet performance targets and contribute to growth metrics

• Experience in documentation using electronic health records (EHR)

• Strong data literacy to forecast service demand and track outcomes

• Excellent interdisciplinary collaboration and team communication

• Patient and family education skills with clear home-program guidance

• On-site availability with strong time-management and scheduling skills

• Commitment to ongoing professional development and pursuing certifications
