Responsibilities
Provide occupational therapy assessments and develop individualized treatment plans.
Deliver on-site OT interventions to improve daily living skills and functional independence.
Monitor patient progress and adjust goals based on measurable outcomes.
Document assessments, treatment plans, and outcomes in compliant electronic health records.
Collaborate with interdisciplinary team to coordinate care and ensure seamless services.
Utilize data and pipelines to forecast patient needs and service demand.
Meet performance targets and contribute to territory/service line growth.
Educate patients and families on therapy goals, home programs, and safety.
Identify opportunities for professional development and pursue relevant certifications.
Participate in quality improvement initiatives to enhance patient outcomes.
Represent the OT program in team meetings and community events to raise visibility.
Experience/Qualifications
OT licensure eligible in applicable state with active license
Licensed Occupational Therapist with 2+ years clinical OT experience
Demonstrated ability to develop and implement individualized treatment plans
Ability to meet performance targets and contribute to growth metrics
Experience in documentation using electronic health records (EHR)
Strong data literacy to forecast service demand and track outcomes
Excellent interdisciplinary collaboration and team communication
Patient and family education skills with clear home-program guidance
On-site availability with strong time-management and scheduling skills
Commitment to ongoing professional development and pursuing certifications
