What Youll Do- Assess patient needs and develop individualized care plans
- Coordinate and monitor the delivery of healthcare services
- Ensure compliance with medical necessity and quality standards

- Participate in multidisciplinary team meetings to optimize care strategies
What You NeedRequired:- Active Social Work (LSW) license or Care Manager certification
- Bachelors Degree in Social Work or related field (BSW) preferred
- Previous experience in case management or healthcare setting
Preferred:- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Knowledge of healthcare regulations and community resources
