We are seeking a compassionate and motivated Occupational Therapist (OT) to join a 126-bed skilled nursing facility in Fremont, California. This is a great opportunity for an OT who enjoys a fast-paced clinical environment, meaningful patient interaction, and helping individuals regain their independence following significant medical events.
The patient population includes Kaiser patients, short-term rehabilitation patients, hip and knee replacement patients, and individuals recovering from neurological conditions. You'll have the opportunity to work with a diverse range of clinical presentations while collaborating with an experienced interdisciplinary team.

Perform comprehensive occupational therapy evaluations and assessments
Develop individualized treatment plans based on each patient's functional needs and goals
Provide skilled OT interventions to improve independence with activities of daily living and functional mobility
Treat patients recovering from orthopedic procedures, including hip and knee replacements
Provide therapy for patients recovering from neurological conditions and other complex medical diagnoses
Monitor patient progress and modify treatment plans as clinically appropriate
Educate patients and caregivers on therapeutic techniques, safety strategies, and continued progress toward independence
Collaborate with physical therapists, nursing staff, physicians, and other members of the interdisciplinary team
Participate in care planning and discharge planning
Maintain accurate, timely, and compliant clinical documentation
Communicate effectively with patients, families, caregivers, and other healthcare professionals
Follow applicable clinical standards, facility policies, and regulatory requirements
Graduate of an accredited Occupational Therapy program
Current California Occupational Therapist license or eligibility for licensure
Strong clinical reasoning and communication skills
Commitment to patient-centered care
Ability to work effectively within an interdisciplinary team
Strong organizational and documentation skills
SNF or rehabilitation experience is a plus
