Title: Personal Care Aide / C.N.A.

Summary: To provide daily care to residents who because of illness or disability cannot perform these functions independently.

Responsibilities: To provide personal care to all residents in a safe, effective and professional manner without violating residents rights.

1. Fulfill resident support plans as outlined

2. To guide and encourage residents in activities of daily living, self help skills, recreation and therapies as directed.

3. Provides routine physical care and activities of daily living to individuals such as, taking vital signs, applying simple dressing, bathing, toileting, oral hygiene,& grooming, etc.

4. Assist with ambulation, transfers, positioning, exercise and wheelchair transport of residents.

5. Maintain all health and safety regulations, OSHA guidelines, mandatory reporting, Incident and reportable disease reporting & exercise universal precautions and maintain clean work areas, including resident bedroom and bathrooms.

6. Participate in ongoing training as required by facility and Department of Human Services. Must keep CPR & First Aide and all other certifications current.

7. Adhere to the policies and procedures of the facility.

8. Cooperate with direction of supervisors and work as a team player

9. Must provide care as assigned, complete assignments accurately and in a timely manner.

10. Observes and report orally and in writing or in any other manner, individual behavior, attitude and physical condition, including changes.

11. Must be able to identify safety hazards and emergency situations and initiate corrective action immediately and report all aspects to shift supervisor accurately. Perform CPR, Heimlich Maneuver, emergency evaluations, other emergency health and safety

12. Must attend all classes and/or meetings as assigned

13. Demonstrate a positive and cooperative attitude and enthusiasm during all tasks.

14. Assume accountability for compliance with Federal, State and Local regulations

15. Must be well groomed, dress according to uniform code and free of infectious diseases at all time

16. Escort individuals to and from various activities, clinics, and appointments and visits on and off the grounds

17. Participate in housekeeping functions within the living quarters which may involve moving furniture equipment.

18. Must be able to perform housekeeping, dietary and activities duties when needed.

19. Must be able to understand the emergency signals, codes and procedures as outlined.

20. Must adhere to attendance and standards of confidentiality.

Qualifications:

1. High School Diploma, GED, or CNA or equivalent

2. Must be 18 years of age or older

3. Acceptable criminal background clearance

4. CPR & First Aide Training

5. Ability to lift and transfer 50 pounds unassisted: Ability to lift up to 175 pounds with assistance

6. Ability to communicate professionally with the resident, resident family, health care professional and co workers.

7. Must be able to read, write, and carry out directions.

8. Excellent attendance and reference history.

9. Ability to work independently and handle emergency/stressful situations.

10. Must have the ability to accept and perform responsibilities as outlined above.

11. Must display a genuine real concern for the well-being of the residents.

I understand that my performance evaluations shall be based on the fulfillment of these responsibilities. I also understand that my failure to perform all or pail of these responsibilities may result in change in employment status, position, progressive discipline and or termination.
