We are seeking a compassionate and dependable Personal Care Attendant (PCA) to support participants at a PACE clinic in Garden Grove, CA. PACE provides comprehensive health and support services to adults age 55 and older with ongoing healthcare needs.
The Personal Care Attendant will provide hands-on personal care, mobility assistance, safety support, and daily living assistance while maintaining the dignity, comfort, and well-being of participants.
Key Responsibilities
Provide personal care and escort assistance according to individual care plans.
Assist participants with:
Grooming and dressing
Showering and bathing
Toileting and peri-care
Feeding
Ambulation and mobility
Assist with safe participant transfers and mobility using appropriate techniques and mechanical equipment when required.
Report changes in participant condition and communicate health and safety concerns to the Day Center Supervisor and PCA Lead.
Follow universal precautions and proper body mechanics to maintain a safe environment.
Participate in social, recreational, and physical activities with participants.
Assist with food service delivery and monitor participants during mealtimes.
Implement interventions and techniques as directed by rehabilitation and nursing staff.
Document services provided on a daily basis.
Collaborate with the interdisciplinary team and support department goals.
Perform other duties and projects as assigned.
Minimum Qualifications
High school diploma or equivalent.
At least 1 year of experience working with frail or elderly individuals required.
At least 6 months of clinical experience as a Personal Care Aide, CNA, LVN, or MA.
Completion of an approved HHA or Medical Assistant program, CNA certification, or LVN license required.
CPR and First Aid certification required or must be obtained within 6 months of hire.
Must complete the PACE-approved Personal Care Aide curriculum upon hire.
Must complete the ServSafe Food Handler certification upon hire.
Ability to work in a fast-paced clinical environment and provide compassionate, respectful care to elderly participants.
Preferred Qualifications
Bilingual in English and one of the following languages: Arabic, Chinese, Farsi, Korean, Russian, Spanish, or Vietnamese.
Experience working with frail, elderly, or medically complex populations.
Strong communication, teamwork, organization, and problem-solving skills.
Basic computer proficiency, including Microsoft Word, Outlook, Excel, and related healthcare systems.
Physical Requirements
Ability to stand, walk, bend, crouch, kneel, reach, and perform other physical activities required for participant care.
Ability to assist with lifting and moving participants and/or equipment weighing more than 50 pounds.
Ability to communicate clearly with participants, families, and healthcare staff.
Ability to read information from computer screens, forms, and other printed materials.
Ability to perform repetitive physical activities while safely assisting frail elderly participants.
