We are seeking a dependable and caring Personal Care Specialist (CNA) to provide hands-on support to patients in their homes while working closely with nurses and the interdisciplinary hospice team.

Position Details

• Location: Wooster, OH

• Job Type: Full-time

• Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m.

• Weekend & Holiday Rotation: Required

• Travel: Local travel throughout the Wooster service area

What You'll Do

As a Personal Care Specialist, you'll help patients remain comfortable and maintain their dignity and independence in the comfort of their own homes. Your responsibilities will include:

• Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility

• Provide compassionate, respectful, and patient-focused care

• Observe patients for changes in condition and promptly report concerns to the appropriate member of the care team

• Accurately document care and services provided using electronic systems

• Promote patient safety, comfort, privacy, and dignity

• Communicate effectively with nurses, caregivers, family members, and other members of the hospice team

• Develop trusting relationships with patients and their loved ones

• Follow the individualized plan of care and hospice policies and procedures

Qualifications

• High school diploma or GED required

• Current Ohio STNA/CNA certification preferred

• At least one year of healthcare or direct patient-care experience preferred

• Basic computer skills and ability to use electronic documentation systems

• Valid driver's license and safe driving record

• Reliable transportation and state-required automobile insurance

• Ability to travel locally between patient homes

• Ability to work in a variety of weather and driving conditions

• Hospice certification (CHPNA) is encouraged within two years of employment
