The Medication History Pharmacy Technician is an essential member of the Department of Pharmacy and plays a critical role in supporting accurate and safe medication use for patients across the health system. This position is responsible for obtaining best‑possible medication histories for Emergency Department patients and admitted inpatients, with a focus on high‑risk populations.
Working both independently and collaboratively, the technician partners closely with pharmacists, nurses, and providers to ensure clear communication and timely updates to the medication profile. The technician conducts structured patient and caregiver interviews, utilizes standardized tools and processes, and reviews external sources of truth to verify and document complete and accurate medication histories in the electronic health record.
This role directly supports the medication reconciliation process by improving the accuracy of prior to admission medication lists, reducing discrepancies, and enhancing interdisciplinary medication management. The technician demonstrates proficiency with automation systems, the EHR, and office software, and maintains all required documentation in accordance with laws, regulations, and departmental policies. Through patient‑centered communication and exceptional customer service, the Medication History Pharmacy Technician contributes to optimal patient outcomes in the acute care setting
Essential Functions and Responsibilities of the Job
Principal Duties and Responsibilities:
- Performs thorough patient medication histories and documents accurately into the electronic medical record.
- Ability to perform the duties and functions of a dispensing Pharmacy Technician.
- Participates in quality assurance programs by maintaining medication history data and reporting any significant medication errors.
- Demonstrates good customer services skills with colleagues and communicates effectively with patients, visitors, and other departments.
- Ability to be at work and be on time.
- Ability to follow company policies, procedures and directives.
- Ability to interact in a positive and constructive manner.
- Ability to prioritize and multitask.

- Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications
Minimum Requirements
Qualifications/Work Experience:
- Minimum three (3) years of experience in an inpatient hospital pharmacy or community pharmacy setting required
Education/Licensure/Certification:
- Current California Registered Pharmacy Technician license required
- Graduated from a Pharmacy Technician Training Program preferred
- Current Certified Pharmacy Technician (CPhT) certificate preferred
