Job Summary and Qualifications As a Pharmacist, you will be responsible for monitoring, evaluating, and making recommendations to assure the appropriate and effective use of pharmaceuticals for patient care. You will interact with patients and healthcare providers as appropriate to define compliance with current standards of practice, as well as assure continuity of services to achieve desired patient outcomes through safe, timely, cost-effective pharmaceutical care.
Responsibilities:
• Interpret medication orders and transcribe to computer system • Accurately and effectively provide medication therapy • Accurately compound and dispense pharmaceuticals • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose • Suggest appropriate cost-effective therapeutic medication alternatives to medical staff • Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges, and promote rational drug therapy selection • Assist with clinical services, such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO renal dosing, and other programs as defined by Pharmacy Leadership • Complete notification reports regarding medication errors and adverse drug reactions
Qualifications
• Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) • Experience: Previous hospital experience - preferred • Licenses and Certifications: Current Pharmacist license with Texas State Board of Pharmacy (TSBPS) • Sterile Products IV Certification through the Accreditation Council for Pharmacy Education (ACPE) • St. Davids North Austin Medical Center part of St. Davids HealthCare is a full-service medical center providing comprehensive medical care at two locations - St. Davids North Austin Medical Center and St. Davids Surgical Hospital.
About St. Davids North Austin Medical Center St. Davids North Austin Medical Center is a 460-bed multi-specialty acute care hospital. It provides advanced womens health services, including maternity and newborn care with Level I, II, and III neonatal intensive care units (NICUs) at the nearby St. Davids Womens Center of Texas. The hospital also offers minimally invasive gynecological surgery and comprehensive breast imaging through Solis Mammography, which is located on-site. Other services include a 24-hour emergency department, pediatric emergency care, and intensive care at St. Davids Childrens Hospital.
