SUMMARY:
The Clinical Coordinator oversees the development, implementation, and optimization of pharmacy clinical services to ensure the safe, clinically effective, and cost-effective medication use across Tucson Medical Center (TMC). Position oversees medication changes to the electronic health record (EHR), and review utilization of medications to identify and support development of medication use guidelines, policies, or protocols regarding optimal medication use. This position supports Pharmacy and Therapeutics Committee to include formulary oversight, facilitation of committees, and development of medication use evaluations, pharmacoeconomic analyses, and best practice recommendations. Additional responsibilities may include strategic partnerships with Service Line leaders, representation on medical center committees, and collaboration on department and inter-professional initiatives.
ESSENTIAL FUNCTIONS:
Reviews new and revised treatment guidelines, order sets and protocols.
Ensures safe and effective use of new or revised treatment guidelines, order sets and protocols.
Provides educational support and competencies as appropriate for all pharmacy staff.
Responsible for preparing all pertinent materials and medication reviews needed for Pharmacy & Therapeutics committee meetings.
Reviews and recommends changes to optimize formulary.
Supervises and supports projects and education provided by all Clinical Pharmacy Specialists.
Assists management with development of department procedures.
Completes required employee evaluations as required by TMCH.
Interprets and clarifies written orders for all types of medications.
Compounds, labels and packages medications and pharmaceuticals; secures storage of narcotics and other controlled substances on a regular basis.
Ensures all charges for dispensed medication orders are captured in hospital's billing system.
Provides medications and drug information relative to circumstances and patient conditions in emergency codes in the hospital.
Communicates drug-related side effects, dosages, and potential complications to health care professionals, patients and/or their representatives.
Remains informed about current developments concerning new drugs and research regarding drug usage.
Promotes effective use of formulary system through drug product selection and therapeutic alternative medication suggestions for non-formulary orders.
Monitors drug therapy and suggest alternative drug therapy when appropriate.
Conducts drug Conducts utilization reviews and prepares drug review criteria for medical staff review and approval.
Applies current knowledge of hospital's investigational drug protocols; ensures maintenance of documentation required to measure patient outcomes.
Applies current knowledge of laws, rules and regulations of local, state and federal agencies which regulate the practice of pharmacy.
Oversees work of Pharmacists and Pharmacy Technicians/Pharmacy Interns participating in the preparation, documentation and dispensing of medications and pharmaceuticals.
Provides department orientation and in-service training for Pharmacy staff; provides input to management for formal performance documentation of Pharmacy personnel.
Adheres to TMCH organizational and department-specific safety, confidentiality, values, policies and standards.
Performs related duties as assigned.
MINIMUM QUALIFICATIONS
EDUCATION: Doctor of Pharmacy degree from an AACP-accredited College of Pharmacy or Bachelor's Degree from an AACP-accredited College of Pharmacy.
EXPERIENCE: Five (5) years of clinical pharmacist experience or completion PGY1 residency with 2 years of additional experience required. Pharmacy residency preceptor experience is also required. Board certification in pharmacotherapy (BCPS) or other pharmacy specialty preferred.
LICENSURE OR CERTIFICATION: Current Arizona Pharmacist license upon hire. Basic Life Support (BLS) and Advance Cardiac Life Support (ACLS). For Oncology: Basic Life Support (BLS) and Advance Cardiac Life Support (ACLS) not required.
