Pharmacist – Contract Assignment

Job Description:

Supplemental Health Care is partnering with a {{ facility type }} in Hudson, Wisconsin to hire an experienced Pharmacist for a contract assignment. Qualifications:

• Active Pharmacist license in the state of Wisconsin
• PharmD or BS Pharmacy degree required
• 1+ year of previous experience in Home Health Or Hospice pharmacy setting
• Strong clinical judgment and attention to detail

Pharmacist Contract Details:

• $2,120 - $2,286 per week*
• AM shift available
• 13-week contract with possibility to extend

Key Responsibilities:

• Medication preparation and dispensing
• Review and interpret medication orders for accuracy and safety
• Monitor patient medication therapy and adjust as needed
• Ensure compliance with all state, federal, and facility regulations
• Maintain accurate records and documentation
• Support pharmacy operations and workflow as needed
