Employment Type:\n \nFull time\n \nShift:\n \nDay Shift\n \nDescription:\n \nJob Title: Pharmacist, Emergency Room\n \nEmployment Type: Full Time\n \nShift: 11am-7:30pm Monday-Friday (with weekend rotation)\n \nLocation: Saint Francis Hospital - Wilmington, DE\n \nPosition Purpose\n \nWe are hiring Pharmacists to support operations and our expanded emergency pharmacy program! We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital. \n \nAs a Pharmacist, you will:\n\n Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding. \n \n\n Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required. \n \n\n Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients. \n \n\n As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care. \n \n\n Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications. \n \n\n Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines. \n \n\n \nMinimum Qualifications: \n\n Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.\n \n\n Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required. \n \n\n Registered pharmacist in Delaware required.\n \n\n Preferred: Experience with Pyxis MedStation, EPIC\n \n\n BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)\n \n\n \nPosition Highlights:\n\n Work/Life balance with flexible schedules.\u202f\n \n\n Retirement Savings Program \n \n\n Free onsite parking.\u202f \n \n\n Our mission and core values are what drive each member of Trinity Health to support each other, communicate openly and respectfully while embracing a culture that nurtures a healing, safe environment for all.\u202f\u202f \n \n\n Referral Rewards Program \n \n\n \nFor more than 90 years, Saint Francis Hospital has served the Wilmington community and Northern New Castle County by providing compassionate health care of the highest quality.
