Job Title: Pharmacist – Home Infusion

Location: Columbia
Pay Rate: $58.06/hour
Schedule: Monday–Friday, 8-hour shifts between 8:00 AM – 6:00 PM
On-Call: Required rotation
Work Type: Onsite (Home Infusion Pharmacy)

We are seeking a skilled Pharmacist to provide clinical pharmacy services in a home infusion setting. This role focuses on medication therapy management, clinical consultation, and collaboration with healthcare teams to ensure safe and effective patient care. This position offers the potential for extension or full-time conversion for the right candidate.

• Provide drug information and clinical guidance to physicians, nurses, and healthcare providers, including drug interactions and contraindications
• Monitor patient therapies and adverse events to ensure safe and effective outcomes
• Support pharmacy operations, including workflow optimization, inventory management, and financial performance
• Assist with implementing processes for medication storage, handling, and compliance
• Deliver clinical and wellness services such as immunizations, diagnostic testing, and disease state management
• Collaborate with insurance providers and medical teams, including participation in third-party audits
• Build and maintain relationships with healthcare professionals in the community
• Support and mentor pharmacy staff to enhance performance and development
• Promote best practices in customer service and patient experience

• Doctor of Pharmacy (PharmD) required
• Active Pharmacist license in the applicable state
• Less than 5 years of professional experience preferred
• Strong clinical knowledge and understanding of medication therapy management

• Advanced problem-solving and decision-making abilities
• Strong knowledge of medical terminology
• Excellent collaboration and teamwork skills
• Ability to manage multiple priorities and deliver results effectively
• Strong digital literacy and systems navigation skills
• Growth mindset with the ability to adapt and support team development

• Onsite role in a home infusion pharmacy setting
• Participation in on-call rotation required
