Overview: The Home Infusion and Specialty Pharmacist performs, coordinates, and supervises pharmacy services including preparation and dispensing of drugs, supplies and equipment, preparation of sterile products as they pertain to providing safe and effective therapies in the home and alternative site settings. Provides clinical assessments, education, care planning and monitoring of responses to therapies and therapeutic outcomes. Hours may vary, and include on-call, depending upon census and program need. Main software used: CareTend, NewLeaf, EPIC, TherigySTM, and Abacus.In the absence of the manager/leader, pharmacists on duty are authorized to assume the duties of the manager/leader to maintain workflow and patient care. These include all of the duties of the Pharmacist-in-Charge (PIC), except those duties restricted to the PIC by local, State, and Federal Law. Responsibilities:
Patient Care & Clinical Monitoring
  • Therapeutic Drug Monitoring: Communicates drug levels and results to physicians, nurses, and staff.
  • Patient Assessment: Evaluates patient data to identify real and potential drug problems.
  • Care Planning: Creates, prioritizes, and sets goals for patient-specific plans.
  • Intervention: Improves how facilities prescribe, administer, and monitor medications. 
Dispensing & Equipment
  • Sterile Compounding: Prepares sterile admixtures safely and dispenses supplies following all laws.
  • Infusion Devices: Programs infusion control pumps and teaches patients, caregivers, and staff how to use them. 
Administration & Billing
  • Clinical Documentation: Completes all records per policy to maintain thorough and accurate charts.
  • Billing & Insurance: Manages patient charging, identifies payers, and obtains prior authorizations before dispensing medications.
  • Compliance: Maintains pharmacy records and ensures operations follow all applicable laws.
Leadership & Consultation
  • Education: Answers drug-related questions for patients, families, and healthcare professionals.
  • Supervision: Supervises pharmacy operations and staff.
  • Quality Improvement: Collects and analyzes Performance Improvement (PI) data for pharmacy functions.
  • Scheduling: Participates in a rotating on-call schedule as a full-time pharmacist.
Qualifications:
  • Certifications: Licensed Pharmacist - Illinois
  • Education: Bachelor's degree in pharmacy is required
  • Work Experience: Pharmacist

Salary Range: The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
