Job Title: Pharmacist – Long Term Care

Location: Horsham
Pay Rate: $64.27/hour
Shift: Second Shift (3:00 PM – 11:30 PM), alternating weekends

We are seeking a dedicated Pharmacist to support long-term care pharmacy operations. This role focuses on accurate medication dispensing, patient safety, and collaboration with healthcare teams to ensure optimal therapeutic outcomes.

• Accept and dispense prescription medications in compliance with all regulations
• Monitor patient medication compliance and identify over-usage patterns
• Ensure prescribed medications are appropriate and safe for patients
• Maintain accurate and confidential patient records and pharmacy documentation
• Provide patient counseling on medication usage, side effects, and interactions
• Offer guidance on non-prescription medications, storage, and dosage
• Support clinical services such as smoking cessation, blood pressure monitoring, and cholesterol management
• Educate patients on proper use of medical devices (e.g., blood pressure monitors, glucose meters)
• Compound medications and prepare sterile solutions as required
• Supervise pharmacy staff including technicians and clerks
• Manage and maintain pharmaceutical inventory and supplies
• Identify drug-related problems and collaborate with physicians and healthcare providers
• Work closely with interdisciplinary teams including nurses, pharmacists, and medical staff

• Doctor of Pharmacy (PharmD) required
• Active Pharmacist license in the state of practice
• Long-term care pharmacy experience preferred
• Experience with AS400 operating system preferred

• Strong clinical knowledge and attention to detail
• Excellent communication and patient counseling skills
• Ability to work effectively in a team-based environment
