Pharmacist (Medication Reconciliation)
The Medication Reconciliation Pharmacist will be based in the Emergency Department and collaborate with interdisciplinary professionals to serve as the designated expert in medication reconciliation. The term "medication reconciliation" is defined by the Joint Commission as "the process of comparing the medications a patient is taking (and should be taking) with newly ordered medications" in order to resolve discrepancies or potential problems. The goals of medication reconciliation are to obtain and maintain accurate and complete medication information for a patient and use the information within and across the continuum of care to ensure safe and effective medication use. The successful candidate will have a unique opportunity to make impactful changes and reach every aspect of the Department and institution, such as but not limited to interviewing patients and families, collecting medication histories, reconciling information received from medication histories, participating in designing and managing patient-centered medication reconciliation systems, educating patients about medications and health care professionals about the medication reconciliation process. A typical day may include interviewing patients, coordinating care with interdisciplinary & external providers, attending safety rounds, precepting pharmacy staff, working with practitioners to resolve acute events, attending medical staff meetings, and educating staff across the hospital on the culture of safety.

Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, an MS in Pharmacy, or a Doctor of Pharmacy required
Active NYS Pharmacist license
Successful completion of PGY1 pharmacy residency within the past seven years, preferred.
Obtain the following American Society of Health System Pharmacist (ASHP) professional certificates within three months of employment
Medication history-taking & reconciliation
Emergency medicine
Obtain BLS & ACLS certification prior to the end of orientation & training period

USD $65.20 - USD $85.28 /Hr.
