IMMEDIATE SUPERVISOR: Director of Pharmacy Services

GENERAL FUNCTION:

The person occupying this position functions as a Pharmacist and serves as the primary therapist for clients in the Aftercare Medication Clinic.

SUPERVISORY RESPOSIBILITY: Pharmacy Tech at same location.

QUALIFICATIONS: KNOWLEDGE AND REQUIRED TRAINING:

The person occupying this position must hold a Bachelor's Degree in Pharmacy and must be licensed by the State of Tennessee as a Pharmacist.

DUTIES AND RESPONSIBILITIES:

1. To dispense medications as prescribed within the Center.

2. To participate in the administration of the pharmacies.

3. To assess the needs and problems of individual clients.

4. Under supervision, to assess the need for and make recommended changes in medication.

5. To participate in discharge planning as needed.

6. To provide client consultation regarding medication instructions as needed.

7. To uphold all local, state and federal laws pertaining to pharmacy.

8. Other duties as assigned.

PERSONAL RELATIONSHIPS: In order to appropriately accomplish his/her duties, the person occupying this position will be required to maintain effective relationships with a wide variety of clinical staff.
