Reports to the Pharmacy Manager. Provides efficient and accurate dispensing of pharmaceuticals in accordance with legal regulatory and accrediting agency standards and in accordance with hospital policies and procedures. Oversees reviews and checks all work of technical and student support staff when that work involves medication preparation andor dispensing. Be on their team.Prepares and dispenses medication orders upon physician request according to established policies procedures and protocols byInterpreting medication orders and transcribing accurately to computerized patient medication profiles and maintaining accurate complete patient medication profiles.Filling package forms such as bottles capsules and unit-of-use packages with measured amounts of medication andor verifying the same work performed by department technical staff.Manipulating needles syringes vials ampules etc. using aseptic technique to prepare intravenous admixtures andor verifying preparation done by department technical staff.Ensures safe appropriate cost-effective medication therapies for patients according to established policies procedures and protocols byMonitoring medication therapy regimens for contraindications drug-drug interactions drug-food interactions drug-laboratory test interactions allergies appropriateness of drug and dose therapeutic duplications and completeness of information.Assisting with the Pharmacy Dosing Service and drug dosing according to hospital protocol policies and procedures.Reading extracting and interpreting information in patient charts accurately.Detecting and reporting suspected adverse drug reactions and medication errors accurately and in a timely manner.Maintaining the formulary of accepted medications by minimizing non-formulary procurements utilizing therapeutic substitution protocols and promoting rational medication therapy selection.Providing consultation and clarification to practitioners suggesting appropriate cost-effective therapeutic alternatives to medical staff as needed.Providing accurate adequate and timely drug information to professional staff patients and families.Documenting all clinical activities and interventions accurately and completely.Conducting research and sharing information gathered in response to questions regarding pharmaceutical therapy.Assures integrity of pharmaceuticals available for use at patient care areas in accordance with established policies and procedures byConducting monthly inspections of assigned areas.Assures maintenance of records pertaining to all phases of pharmaceutical procurement dispensing and use in accordance with hospital legal and regulatory requirements byRecording necessary information and filing records as appropriate.Reviewing documentation of medication administration as assigned.Verifying the work of technical support staff in these areas.Assures proper charging of pharmaceutical products and services byAccurately processing entries into the hospital computer system.Supervising the work of departmental support personnel engaged in these activities.Performs other functions to maintain personal competence and contribute to the overall effectiveness of the department byInitiating andor coordinating special projects as agreed upon with a supervisor pertaining to skills and abilities as indicated below are normally acquired through the successful completion of a Bachelors degree in Pharmacy or a Pharm D degree from an accredited school of pharmacy or successful completion of the foreign pharmacy equivalency exam and licensure as a registered pharmacist or eligible for licensure in the State of Indiana.Knowledge & SkillsDemonstrates the interpersonal and communication skills both verbal and written necessary to interact effectively with physicians hospital staff and other health care professionals to assure optimum pharmaceutical care.Demonstrates a thorough knowledge of policies procedures and standards for the provision of pharmaceutical services.Working ConditionsMay be exposed to potentially hazardous substances.Physical
