Responsible for performing and documenting patient pharmacy clinical interventions, education, and counseling that pertains to medication management and safety.
Interprets, evaluates and reviews of physician inpatient orders and outpatient prescriptions in order to prepare, calculate, compound, admix and dispense appropriate medication and dosages consistent with optimum patient care.
The interpretation, evaluation and review must include but is not limited to:
(I.E., pharmacist is responsible to ensure the dispensation of the right medication, at the right dosage, for the right medical indication, at the right time, to the right patient.)
Engages in practices that promote and advance professional practice, education, patient safety, and collaboration with other health care practitioners.
