About the role

This is a dual clinical and relationship role. The Consultant Pharmacist delivers the clinical oversight required of an LTC pharmacy—drug regimen reviews, medication cart audits, and survey-readiness support—while also acting as the dedicated point of contact between the pharmacy and its facility partners. Beyond servicing existing accounts, this role carries direct responsibility for growing CRPS’s presence in Vermont by building relationships and developing new facility partnerships. The ideal candidate blends strong geriatric clinical judgment with the interpersonal skills and business sense to keep facilities satisfied and the territory growing.

Key Responsibilities

Facility Liaison & Account Management

• Serve as the primary point of contact between CRPS and assigned LTC facilities, owning the overall relationship and service experience.
• Build and maintain trusted relationships with facility leadership, including administrators, directors of nursing, charge nurses, and medical directors.
• Conduct regular on-site visits and proactive check-ins to anticipate needs and reinforce the partnership.
• Lead onboarding for new facilities, including portal setup, ordering workflows, delivery expectations, and introductions to the pharmacy team.
• Coordinate closely with pharmacy operations, fulfillment, and billing teams to resolve service issues quickly and follow through to resolution.
• Deliver staff in-services and training on the pharmacy portal, medication ordering, cycle fill, emergency kits, and medication-safety best practices.

Clinical Consultant Pharmacist Services

• Drug Regimen Review (DRR): Perform monthly—or more frequent as required—medication regimen reviews for residents in accordance with 42 CFR 483.45, Vermont Board of Pharmacy requirements, and facility policy.
• Identify and document medication irregularities, potential adverse drug reactions, drug interactions, therapeutic duplications, and unnecessary or high-risk medications.
• Make evidence-based recommendations to prescribers and nursing staff to optimize therapy, reduce polypharmacy, and support psychotropic/antipsychotic gradual dose reduction (GDR) efforts.
• Communicate findings and recommendations clearly, track prescriber responses, and document resolution and follow-up.
• Provide drug-information support to facility staff, including guidance on administration (e.g., crush/no-crush), monitoring, and medication safety.

Cart Audits, Storage & Compliance

• Conduct routine medication cart audits and medication-storage inspections at each facility.
• Verify proper labeling, storage conditions, security of controlled substances, and disposal of expired or discontinued medications.
• Audit emergency kits (E-kits) and after-hours supplies for integrity, accuracy, and proper documentation.
• Ensure compliance with federal LTC regulations, DEA requirements, state board of pharmacy rules, and HIPAA.
• Support each facility’s survey readiness and QAPI (Quality Assurance and Performance Improvement) program, documenting deficiencies and corrective actions.

Customer Satisfaction & Service Quality

• Own facility satisfaction as a core measure of success; proactively gather feedback and act on it.
• Monitor service metrics and address concerns before they escalate, serving as the facility’s advocate inside the pharmacy.
• Partner with the marketing and account-management team to maintain consistent, professional communication across all accounts.

Business Development – Vermont Market

• Identify, pursue, and win new LTC facility partnerships across Vermont, helping establish and grow CRPS’s footprint in the state.
• Represent CRPS at industry events, association meetings, and facility presentations; articulate the pharmacy’s clinical and service capabilities.
• Collaborate with marketing/sales and leadership on proposals, capability presentations, and contract discussions.
• Support Vermont Medicaid (Green Mountain Care) and payer enrollment readiness as it relates to facility onboarding and go-live.
• Provide market intelligence on competitors, prospects, and regulatory developments affecting LTC pharmacy in Vermont.

Qualifications

Required

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program.
• Active, unrestricted pharmacist license in Vermont, or eligibility to obtain NY licensure promptly upon hire.
• Minimum of 2–3 years of pharmacy practice experience.
• Working knowledge of federal and state LTC regulations (42 CFR 483 and related F-tags), DEA controlled-substance rules, and HIPAA.
• Valid driver’s license, reliable transportation, and willingness to travel regularly to facilities throughout Vermont.

Preferred

• Consultant pharmacist experience in long-term care or skilled nursing settings.
• Board Certified Geriatric Pharmacist (BCGP) or comparable certification.
• Established relationships within the Vermont LTC community.
• Multistate licensure (NY and VT) and prior account management or business development experience.

Knowledge, Skills & Abilities

• Strong clinical knowledge of geriatric pharmacotherapy and medication safety.
• Excellent interpersonal and relationship-building skills, with the ability to influence prescribers and partner with facility leadership.
• Self-directed and highly organized, able to independently manage a field territory and competing priorities.
• Clear, professional written and verbal communication.
• Comfort with pharmacy software, dashboards, and digital ordering portals.
• A service-oriented, growth-minded approach that balances clinical rigor with customer satisfaction.

Working Conditions & Physical Requirements

• Field-based role with frequent travel to LTC facilities; remaining time split between facility sites, remote work, and pharmacy operations.
• Ability to stand and walk for extended periods during facility visits and cart audits, and to occasionally lift or move materials up to 25 pounds.
• Work is performed in clinical and office environments consistent with healthcare and pharmacy settings.
