Summary:The Clinical Pharmacist position is responsible for providing clinical pharmacy services to the highest standard of pharmaceutical care principles for each patient within their area of responsibility. Clinical areas of coverage include:
The position is responsible for therapeutic assessment, interventions and plans for individual patients as well as patient populations. For Hospice patients, the Pharmacist assumes prescriptive authority, managing the daily comfort needs and terminal pain control. The Pharmacist works closely with the Hospice nurses who report patient status. Documentation for the Hospice patient encounters are documented in both the Hospice EHR (HomeCare HomeBase) and the Pharmacy EHR (Epic Hyperspace and Willow Ambulatory). The Pharmacist is a valued member of the Interdisciplinary Care Team and assumes responsibility for medication related outcomes.
