Job Title: Pharmacist ONCOLOGY PHARMACIST
Pay range: $## - $## an hour
Registered Pharmacist currently licensed in New York State. Completion of PGY2 Oncology Residency and/or Board Certification in Oncology (BCOP) OR 1-5 years or more time spent preparing/evaluating chemotherapy regimens required. Familiarity and experience in developing processes and procedures to meet USP 797, USP 800 and all other regulatory requirements for compounding sterile preparations.
Chemotherapy Inventory and Procurement a. Assists Pharmacy Buyer in maintaining accurate patient list with adequate par levels for all chemotherapy agents (updates on weekly basis) to minimize waste b. Routinely reviews areas of the pharmacy inventory to ensure adequate stock levels/arrangement and removes drugs that are outdated or no longer utilized. Promotes use of the formulary system and works within its confines.
Reviews all chemotherapy order sheets for accuracy and optimization a. Compares to National Comprehensive Cancer Network guidelines or study protocol and attaches reference to order sheet b. Coordinates with provider to round to the nearest vial sizes to reduce waste c. Ensures stability of diluent chosen d. Ensures order within recommended concentration range e. Defines recommended beyond-use date and informs Information System Pharmacist to add to Drug Dictionary as label comment f. Monitors life-time doses
Contacts and confers with Oncology Department providers/nursing staff if there are any questions or irregularities regarding any medication orders.
Coordinate plan with Oncology Department for prescribing of chemotherapy and medication reconciliation
