Position Summary:
Provides prescribed medications, drugs and other pharmaceuticals as needed for adequate patient care according to professional standards and practices.
Responsibilities:
- Compounds, labels and packages medications and pharmaceuticals.
- Advises patients and family on use of various medications including dosage, side effects and composition.
- Ensures secure storage of narcotics and other controlled substances on a regular basis.
- Checks stock on a regular basis to identify and reorder outdated stock and ensure that stock is maintained in accordance with manufacturer requirements.
- Assists in preparing required reports by compiling information.
- Adheres to all hospital and departmental policies and procedures, objectives, quality assurance programs as well as safety, environmental and infection control standards in the performance of work duties.
- Maintain professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
- Attend meetings as required and participates on committees as directed. Attends and serves on professional/civic service organizations as hospital representative.
- Perform other related duties as assigned or requested.
- Monitor anticoagulation service patients per protocol.
- Monitor medication doses, interactions, and other critical pharmacokinetic effects and report information to physicians and other patient care staff as appropriate. Perform interventions per protocol.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.
- Performs other duties as assigned.
Qualifications:
Minimum Education:
None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience:
Completion of internship.
Licensure Requirement:
Current TN pharmacist license.
