Pharmacist - Travel Job

Minimum Requirements:

• Active Pharmacist license for South Carolina required.
• Experience working in a hospital pharmacy setting required.
• Demonstrated competency with sterile product preparation and parenteral labeling.
• Experience managing controlled substances and maintaining required records.
• Experience with medication procurement, inventory management, and monthly pharmacy inspections.
• Clinical experience with antimicrobial stewardship, including interpretation of culture results and recommendations for common infections (UTI, pneumonia, SSTI) required.
• Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring.
• Experience performing medication reconciliation for discharge medication lists and providing discharge counseling to patients and families.
• EPIC electronic health record experience preferred.
• Minimum 6 months of pharmacist experience preferred.

Assignment Details

• Facility Type: Hospital
• Shift: Day (5x8)
• Duration: 13 Week assignment
• Must perform tasks under general supervision of Pharmacy Director and collaborate with multidisciplinary teams.
• Responsibilities include sterile preparation, dispensing, controlled substance management, medication reconciliation, formulary support, and participation in Pharmacy & Therapeutics and quality assurance activities.
