## Job Title: Pharmacist

### Job Summary:

We are seeking a knowledgeable and licensed Pharmacist to provide safe, effective, and patient-centered pharmaceutical care. The Pharmacist will verify and dispense medication orders, monitor medication therapies, provide drug information to healthcare providers, identify and resolve medication-related issues, ensure regulatory compliance, and collaborate with the healthcare team to optimize patient outcomes.

### Requirements:

* Graduate of an accredited School of Pharmacy required
* Doctor of Pharmacy (PharmD) degree preferred
* Active Pharmacist license in the state of practice
* All state-required pharmacist licensures and certifications
* Current Basic Life Support (BLS/CPR) certification
* Advanced Cardiac Life Support (ACLS) certification required
* Pediatric Advanced Life Support (PALS) certification required
* One year of pharmacy experience preferred
* Inpatient clinical pharmacy experience preferred
* Strong clinical knowledge, communication, and problem-solving skills

### Key Responsibilities:

* Review, verify, process, and dispense medication orders accurately and in a timely manner.
* Monitor medication therapies to ensure safety, effectiveness, and optimal therapeutic outcomes.
* Evaluate medication orders for appropriate indication, dosage, route, frequency, allergies, and potential drug interactions.
* Provide drug information and medication counseling to physicians, nurses, and other healthcare professionals.
* Conduct medication utilization reviews and participate in quality improvement initiatives.
* Participate in clinical pharmacy services, medication management activities, and interdisciplinary patient care rounds.
* Ensure compliance with hospital policies, regulatory standards, and pharmacy best practices.
* Assist with formulary management, medication procurement, inventory control, and cost-saving initiatives.
* Identify medication safety concerns and recommend process improvements.
* Supervise and provide guidance to pharmacy technicians and support staff.
* Perform other job-related duties as assigned.
