    The PTC Pharmacist works under the supervision of the System Director of Retail Pharmacy. The PTC Pharmacist is a highly skilled and patient-centered clinician responsible for optimizing medication therapy for patients with chronic conditions through a collaborative practice agreement (CPA). This role integrates advanced clinical practice while providing safe, effective, and evidence-based pharmacologic interventions for patients with a broad range of disease states. Through direct patient care, interdisciplinary collaboration, and leadership in medication management programs, this specialist ensures optimal therapeutic outcomes, enhances patient safety, and advances pharmacy practice.

    QUALIFICATIONS:

    • Education Postgraduate degree in Pharmacy RPh or PharmD required.
    • A certificate of medication therapeutic plan authority is preferred.
    • Required Experience/Skills Licensure: Current Pennsylvania Pharmacy License in good standing with the Board of Pharmacy is required.
    • Preferred Experience/Skills Pharmacy residency or board certification preferred.
    • Ambulatory care residency preferred.
    • Excellent interpersonal skills in order to work with physicians and nurses for drug utilization.
    • Telephone manners are of a nature that promotes a positive image of the Pharmacy Department.
