We are seeking a highly motivated and experienced Pharmacy Clinical Coordinator / Pharmacist in Charge (PIC) to join our healthcare team. This is a permanent position that offers an exciting opportunity to contribute to and drive the success of our hospital pharmacy operations. The successful candidate will be responsible for managing all aspects of the pharmacy department, including medication management safety, 340B program management, regulatory compliance, and more.

Responsibilities:

1. Oversee and manage the daily operations of the hospital pharmacy, ensuring efficiency and safety at all times.
2. Utilize knowledge of Pharmacy Informatics to optimize the use of Electronic Health Records (EHR) and Meditech.
3. Ensure compliance with the California Board of Pharmacy and Joint Commission standards.
4. Lead the 340B program management, including working with the Sentry 340B Medication Use Evaluation.
5. Manage the Alaris CQI and Alaris Drug Library, utilizing the CareFusion Knowledge Portal.
6. Develop and implement policies and procedures to ensure best practices in medication management.
7. Conduct regular medication use evaluations and implement changes as necessary.
8. Oversee inventory management and pharmacy purchasing to ensure optimal stock levels.
9. Develop clinical decision support order sets to improve patient outcomes.
10. Maintain readiness for Joint Commission reviews and inspections.
11. Provide leadership and guidance to pharmacy staff, fostering a positive and productive work environment.

Qualifications:

1. Must have a PharmD degree from an accredited institution.
2. Must hold a valid and active Pharmacist license in the state of California.
3. Minimum of 5 years of experience in a hospital pharmacy, with at least 2 years in a leadership role.
4. Extensive knowledge and experience with Pharmacy Informatics, EHR, Meditech, and 340B program management.
5. Experience with Alaris CQI, Alaris Drug Library, and CareFusion Knowledge Portal.
6. Strong understanding of medication management safety and regulatory compliance.
7. Proven experience in inventory management and pharmacy purchasing.
8. Excellent leadership and team management skills.
9. Strong communication and interpersonal skills.
10. Proactive with a strong commitment to continuous improvement.
11. Must have a strong understanding and knowledge of California Board of Pharmacy Compliance and Joint Commission readiness.
