Job Title: Pharmacy Lead
LEAD ONCOLOGY PHARMACIST
Must have LEAD experience
Must have ONCOLOGY experience

Pay ranges: $##-$## an hour

Current New York State Pharmacy license and registration required.
Completion of PGY2 Oncology Residency and/or Board Certification in Oncology (BCOP) is preferred.
In lieu of residency/board certification, at least 1 years of time spent preparing/evaluating chemotherapy regimens is required.
Familiarity and experience in developing processes and procedures to meet USP 797, USP 800 and all other regulatory requirements for compounding sterile preparations is required.

Develops, coordinates, and maintains records for employee training for chemotherapy preparation
Prepares and maintains any policies specific to chemotherapies and/or biologic medications
Works with pharmacy and infusion center staff to optimize workflow and limit delays to patient care
Chemotherapy Inventory and Procurement
a. Assists Pharmacy Buyer in maintaining adequate medication inventory to minimize drug waste
b. Routinely reviews areas of the pharmacy inventory to ensure adequate stock levels/arrangement and removes drugs that are outdated or no longer utilized.
Reviews all chemotherapy order sheets for accuracy and optimization
a. Compares to National Comprehensive Cancer Network guidelines or study protocol and attaches reference to order sheet
b. Coordinates with provider to round to nearest vial sizes to reduce waste
c. Ensures stability of diluent chosen
d. Ensures compounded medications are within recommended concentration ranges
e. Monitors life-time doses for medications with cumulative dose limits (e.g. doxorubicin)
New formulary medications used in the Oncology/Infusion Center
a. Assists Pharmacy Buyer in completion of the New Drug Checklist
b. Develops and implements education for pharmacy and oncology staff for new medications
c. Identifies necessary drug-specific alerts (e.g. requires use of in-line filter) and informs Information System Pharmacist or Pharmacy Buyer to add to Drug Dictionary as label comment
Contacts and confers with infusion center providers/nursing staff if there are any questions or irregularities regarding medication orders.
Assists prior authorization staff to ensure that therapies will be covered by patients insurance
Upon request, assists the infusion center staff with patient counseling and medication reconciliation
