Pharmacist Pharmacist In Charge (PIC) — Home Infusion Pharmacy
Location: Near College Station, TX
Position Type: Full-Time | On-Site
This is an established, growing home infusion pharmacy serving patients across the Brazos Valley, Greater Houston, and rural East and South Texas. We are seeking an experienced, hands-on Pharmacist-in-Charge to lead our pharmacy operations, uphold clinical and regulatory excellence, and help build a high-performing team as we continue to scale.
The Pharmacist-in-Charge (PIC) holds full operational, clinical, and regulatory responsibility for the pharmacy as required by the Texas State Board of Pharmacy. The PIC ensures the safe, accurate, and timely preparation, verification, and dispensing of sterile and non-sterile compounded medications for home infusion patients, while directing day-to-day operations, leading staff, and serving as the pharmacy’s primary point of accountability for compliance, quality, and patient safety.
This is a leadership role for a pharmacist who takes ownership: someone who anticipates issues, drives operational discipline, mentors the team, and represents Vital Care of College Station to prescribers, partners, and the broader referral community.
Pharmacy Operations & Day-to-Day Oversight
• Operational accountability. Hold full responsibility as Pharmacist-in-Charge for daily pharmacy operations, ensuring safe, high-quality infusion therapy services across therapies such as immunoglobulin (IVIG/SCIG), anti-infectives, specialty biologics, nutrition support (TPN), pulmonology therapies, alpha-1 antitrypsin therapy, and anti-inflammatory therapies.
• Workflow leadership. Direct and optimize pharmacy workflows to enhance efficiency, including speed-to-service and post-therapy follow-up.
• Environment. Maintain a clean, organized, and patient-friendly environment that reflects our standards of excellence.
• Hands-on support. Oversee order verification, clinical review, and patient education, stepping directly into operations as volume and staffing require.
Clinical Practice & Patient Care
• Review, interpret, and clinically verify infusion therapy prescriptions for appropriateness, accuracy, and compatibility.
• Conduct ongoing clinical assessments and therapy monitoring for home infusion patients.
• Provide drug information and therapy support to prescribers, nursing teams, and patients/caregivers.
• Educate patients and caregivers on medication administration, storage, handling, and safety requirements.
• Ensure seamless patient care coordination in collaboration with nursing, reimbursement, and customer service teams.
• Document all clinical interventions, assessments, and communications in the EMR.
Sterile & Non-Sterile Compounding
• Oversee and verify sterile compounding activities in accordance with USP <797>, ACHC standards, and company SOPs.
• Perform final product checks before dispensing to ensure accuracy, sterility, and labeling compliance.
• Ensure adherence to best practices in cleanroom workflow, garbing, beyond-use dating, and environmental monitoring.
Regulatory, Quality & Compliance
• Serve as PIC of record, ensuring compliance with Texas State Board of Pharmacy requirements, federal regulations, ACHC accreditation standards, USP <797>, and internal policies.
• Maintain all documentation and support regular audits and accreditation surveys for quality assurance.
• Stay informed of evolving laws, regulations, and clinical advancements in infusion therapy.
• Uphold patient confidentiality and comply with HIPAA regulations at all times.
• Support quality initiatives related to clinical excellence, patient safety, and documentation accuracy.
Inventory, Ordering & Supply Chain Management
• Accurately dispense, order, and maintain inventory of medications and infusion supplies.
• Identify and communicate potential medication shortages, incompatibilities, or substitution needs.
• Ensure appropriate storage, handling, and system tracking of specialty biologics and infusion products.
Team Leadership, Mentorship & Culture
• Recruit, onboard, train, and lead a high-performing pharmacy team of pharmacists and technicians.
• Address staffing, performance, and professional-growth needs as the pharmacy scales.
• Promote a culture of continuous improvement, ownership, compliance, and clinical excellence.

Provider, Partner & Stakeholder Collaboration
• Serve as the key liaison between pharmacy operations, referring providers, clinic administrators, vendors, and internal leadership.
• Collaborate with referring providers and clinical staff to resolve prescription issues and ensure smooth coordination of care.
• Provide clinical consultation to physicians and care teams across partner organizations.
• Represent the College Station location in industry forums and regional referral networks.
Additional Duties
• Perform additional responsibilities as assigned and consistent with our policies.
• Provide or arrange staffing coverage for PTO, high-volume periods, or emergent operational needs.
• Maintain readiness to support urgent requests and adapt to changing priorities; occasional evenings, weekends, or on-call duties may be required.
• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
• Active, unrestricted pharmacist license in the State of Texas (or eligibility for reciprocity/transfer); must qualify to serve as Pharmacist-in-Charge under Texas State Board of Pharmacy rules.
• 5+ years of pharmacy experience required; direct experience in home infusion, sterile compounding, or specialty pharmacy strongly preferred.
• Demonstrated success in pharmacy leadership, PIC, or startup/build-out environments.
• Specific clinical experience with infusion therapies (e.g., immunoglobulin, TPN, specialty biologics, alpha-1 antitrypsin) highly desired.
• Strong knowledge of pharmacy operations, clinical protocols, compounding standards (USP <797>), and infusion device operation.
• Experience with home infusion pharmacy software (e.g., CareTend, CPR+) preferred.
• Deep knowledge of federal and state pharmacy regulations and ACHC accreditation.
• Excellent communication, time management, and problem-solving skills.
• Ability to work independently, manage changing priorities, and respond to urgent operational needs.
