Description:

The St. Joseph Mercy Hospital Pharmacy Practice Residency Program is an organized, directed, postgraduate program intended to develop a highly trained pharmacist with advanced knowledge and skills in the application of drugs and drug therapy to pharmacy practice. The Pharmacy Practice Resident will perform all staff pharmacist functions on a regularly assigned basis.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Performs Staff Pharmacist functions on a rotating basis.

Drug therapy monitoring of assigned patients and communicating relevant findings and recommendations to other health care professionals who are also responsible for the patient's care and documentation of outcomes.

Development of patient-specific drug therapy management plans and provision of oral and written consultations with other health care professionals regarding drug therapy selection and management.

Patient education and counseling regarding drug therapy and drug-related disease prevention.

Preparation of medication histories for patient's, for the patient's medical record or other databases.

Monitoring, detecting, reporting, and managing adverse drug events.

Participation in medication-use evaluations and other quality improvement programs.

Initiation of and participation in drug and drug-related research.

Provision of accurate and comprehensive information about drugs and patient-specific drug information to other pharmacists and health care professionals.

Participation in staff development activities. Preceptor for university affiliated pharmacy externship students.

Medication use policy development and implementation.

Provision of patient-care pharmacist services every third weekend and as assigned.

Participation in appropriate committees related to medication use.

Provides quality patient care by considering the age specific, development and cultural needs through competent clinical practice. Demonstrates unit/area competencies.

OTHER FUNCTIONS AND RESPONSIBILITIES

Performs other duties as assigned.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

Education:

Graduate from an accredited college of pharmacy. Advanced degree in clinical area of pharmacy practice (Doctor of Pharmacy, Master's degree, etc.) preferred.

Experience:

Previous hospital pharmacy experience preferred.

Certification, Registration, or Licensure:

Licensure as a pharmacist in the state of Michigan by October (preferred) or January of the residency year.

REQUIRED SKILLS AND ABILITIES

Interpersonal skills necessary to effectively communicate drug therapy information to all health team members, patients, and administrators/managers.

Has a demonstrated working knowledge of clinical pharmacokinetics (mathematic modeling of a drug's absorption, distribution, metabolism, and excretion in a patient).
