Job Summary and Qualifications

This is a one-year position for a recently graduated pharmacist with a Doctor of Pharmacy degree. This position will be responsible for completing the requirements of the residency program as outlined in the Pharmacy Residency Policy Manual, which includes, but is not limited to rounding with various medical teams (Internal Medicine, Surgery, etc.), providing pharmaceutical care to the patients on whichever team they are assigned to, completion of a residency project and medication use evaluations. The resident is also expected to perform all of the functions listed on the clinical pharmacist's PDQ including pharmacokinetic consults, antimicrobial therapy consults, evaluation of appropriateness of pharmacotherapy, preparation of drug therapy monographs for the P&T committee, providing drug information to health professionals and providing patient education. Complies with the Memorial Health Compliance Program, related policies and procedures and the Code of Business Practices; reports instances of perceived and actual non-compliance to Team Leader or through other reporting mechanisms.

  • Providing direct pharmaceutical care to improve patient outcomes. Evaluation of drug therapy regimens, including recommending changes in current regimens to health professionals, developing and implementing monitoring plans, redesigning drug therapy regimens based on evaluation of monitoring data. Participate in interdisciplinary patient care rounds.
  • Function as a staff clinical pharmacist. Prepare and dispense medications using appropriate techniques according to hospital and departmental policies and procedures. Participate in the management of medical emergency codes.
  • Providing concise timely drug information upon request from health care providers or patients. Provide direct patient education on drug therapy to patients. Write newsletter articles that provide pertinent drug-use information for health care professionals. Provide in service education to physicians, nurses, and other clinical practitioners.
  • Precept Pharm.D. students on clinical rotations in the hospital. Responsible for coordinating student activities and serving as a clinical resource and mentor for Pharm.D students.
  • Works to minimize errors in the medication use process by utilizing S.T.A.R. (Stop, Think, Act and Review) and other error prevention techniques, especially when dealing with a situation that is out of policy, procedure, or plan, or any other high- risk situation. Strictly follow special handling procedures related to high-risk, high-alert medications and patient populations.
  • Participate in the ongoing revision of the health-system's formulary; participate in the modification of the drug use evaluation program in the hospital; participate in the hospital's process for assessing, managing, preventing, and reporting adverse drug events and medication errors.
  • Performs other related responsibilities as required.

What qualifications you will need:

  • Doctor of Pharmacy (PharmD) Required.
  • Must be a candidate for graduation with Doctor of Pharmacy Degree and obtain degree within 30 days from hire date.
  • GA Pharmacist license as outlined in the Pharmacy Residency Policy Manual.
