Under direct and indirect supervision, the Pharmacy Technician / Patient Care Representative communicates with patients about therapy progress, coordinates refills, updates medication profiles, and supports pharmacists by obtaining and entering laboratory values. The position focuses on high-quality customer service, accurate data entry, and effective collaboration with clinical and operational teams, all within a non-patient-facing environment that offers room for growth and advancement.

Responsibilities

  • Maintain the team schedule for patient follow-up, refill assessments, and deliveries to ensure timely and accurate service.
  • Participate in daily team huddles to prioritize and coordinate the processing, compounding, and delivery of medications and supplies.
  • Communicate with patients to review current inventory and determine appropriate refills for existing orders, ensuring continuity of therapy.
  • Anticipate patient care and supply needs to maintain appropriate inventory levels and prevent interruptions in treatment.
  • Report identified non-compliance, process orders, and print labels and packing lists in a timely and accurate manner.
  • Ensure timely and appropriate documentation in progress notes for all patient interactions and activities.
  • Communicate effectively with patients, caregivers, and clinicians involved in the patient's care, discussing therapy progress and documenting updates in progress notes.
  • Forward all patient concerns and clinical questions to the team pharmacist for review and follow-up.
  • Coordinate with the Warehouse Coordinator to ensure patients receive adequate supplies and timely deliveries.
  • Update patient medication profiles in the electronic medical record (EMR) and present them to the team pharmacist for review.
  • Follow up with physician offices as appropriate within the scope of practice to support ongoing patient care and prescription management.
  • Notify the reimbursement team of patient hospitalizations, prescription changes, and changes in patient status to support accurate billing and coverage.
  • Assist pharmacists in obtaining laboratory values, enter lab results into the EMR, and forward them to the ordering physician as appropriate.
  • Provide delivery representatives with patient pick-up forms and assist in scheduling the pick-up of equipment and medical waste.
  • Perform all other duties assigned by the supervisor or manager to support pharmacy operations and patient care.
  • Maintain a complete understanding of confidentiality requirements regarding proprietary information and patient/client care information.
  • Comply with all federal and state laws related to the confidentiality of protected health information (PHI) and electronic protected health information (EPHI).
  • Follow HIPAA guidelines and protect readily identifiable PHI in all communications and documentation.

Essential Skills

  • Minimum of 6 months of experience as a pharmacy technician or in a closely related role.
  • Good working knowledge of pharmacy procedures and workflows.
  • Working knowledge of home infusion, specialty infusion, and durable medical equipment (DME).
  • At least 2 years of computer data entry experience with a strong focus on accuracy and speed.
  • Demonstrated customer service experience with a strong track record of customer satisfaction.
  • Strong computer skills, including proficiency in Microsoft Word, Excel, and PowerPoint.
  • Experience with pharmacy support functions such as prior authorization, retail pharmacy, fulfillment, and inventory management.
  • Ability to update and maintain medication profiles and enter clinical data in an electronic medical record (EMR) system.
  • Exceptional verbal and written communication skills for effective interaction with patients, caregivers, clinicians, and internal teams.
  • Strong interpersonal skills to build collaborative working relationships across departments.
  • Strong organizational and process skills, with the ability to manage multiple tasks and priorities.
  • Ability to work well under pressure, meet deadlines, and complete assigned projects on time.
  • Strong critical thinking and problem-solving skills to address operational and patient care issues.
  • Exceptional attention to detail and demonstrated ability to produce accurate, high-quality work.
  • Proven performance history in a related field, demonstrating reliability and consistent results.

Additional Skills & Qualifications

  • Certified Pharmacy Technician (CPhT) credential is highly valued.
  • Experience in closed-door pharmacy operations, particularly in home infusion settings.
  • Background in retail pharmacy, prior authorizations, fulfillment, or inventory management.
  • Familiarity with healthcare documentation standards and best practices for progress notes.
  • Experience working with reimbursement teams or understanding of insurance and coverage processes.
  • History of successfully contributing to process improvements and operational efficiency.
  • Demonstrated ability to adapt to increased volume and changing workflows in a growing pharmacy environment.

This position is based in a closed-door pharmacy environment that supports home infusion and related services, with no direct patient-facing interactions. The typical schedule runs between 8:30 a.m. and 6:00 p.m., with an 8-hour shift starting between 8:30 a.m. and 9:30 a.m., providing a structured daytime work schedule.
