Working under the guidance of a licensed pharmacist, the Medication History Specialist plays a vital role in supporting safe, high-quality patient care by obtaining accurate home medication information directly from patients and other reliable sources. This position is responsible for carefully documenting medication histories in the computer system, researching and resolving discrepancies, and collaborating with pharmacies, external databases, primary care clinics, and other resources to create the most complete and accurate medication profile possible. Qualifications:
· Must be a Certified Pharmacy Technician and registered with the Board of Pharmacy
· Minimum of 2 years of pharmacy technician experience required
· Computer proficiency required
· 3 years of pharmacy technician experience preferred, but not required
