    Hours: FT Night Shift 7pm-7am

    Job Summary:

    Provides direct patient care and performs selected therapeutic interventions and diagnostic procedures under the direction of the Registered Nurse.

    Accountabilities:

      1. Provides safe and efficient direct patient care with consideration for age-specific accommodations.
    • a. Provides/assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention.
    • Activities of daily living including hygiene
    • Nutrition and hydration
    • Comfort
    • Mobility and repositioning
    • Elimination
    • Skin care
    • Ambulation assistance
    • b. Recognizes and reports abnormal observations and/or changes in the patient's condition/progress to the Registered

    Nurse in a timely manner.

    • c. Responds to patient/family requests in a timely manner utilizing excellent customer service skills.
    • d. Responds appropriately in a critical situation, i.e.: code, respiratory compromise, falls, seizures.
    • e. Demonstrates appropriate skill in utilization of the computer for documentation, point of care results, data retrieval,

    and communication, ensuring that no unapproved abbreviations are used.

    • f. Assists in the admission, transfer and discharge process.
    • Orients patient to room, call bell, and routine
    • Assists with packing and unpacking belongings
    • Assists in the transporting of patients
    • g. Demonstrates competence in technical skills and selected therapeutic interventions and diagnostic procedures.
    • Vital signs
    • Intake and output
    • Point of care testing (Accudata, nasal swabs)
    • Bladder scanner
    • Hot and cold applications
    • Abdominal binders, antiembolism stockings, and sequential compression sleeves.
    • Support and mobility devices/orthotics, prosthetics, and gait training equipment
    • Specimen collection via mobile lab according to Specimen Collection Manual and Reference Laboratory Manual
    • Surgical preps (clips, scrubs & wipes).
    • Patient transport as necessary
    • Phlebotomy (venipuncture and capillary) per unit requirement
    • 12 lead EKG
    • h. Demonstrates initiative, self-direction and accountability in completion of assigned responsibilities in accordance with

    department needs.

      1. Collaborates with patients, families, and health care team members in order to achieve excellence in patient care.
    • a. Utilizes effective interpersonal skills including interaction through understanding, talking, hearing, seeing and listening.

    Accountabilities

    • b. Participates in patient/family education as appropriate to level of training and practice, and with consideration for the

    patient's ability to understand and respond

    • c. Consistently and continuously communicates, pertinent information regarding changes in patient status and patient care

    to patient care team members including skin breakdown, vital signs, intake and output, etc.

    • d. Demonstrates initiative in offering assistance to other team members
    • e. Uses interpersonal communication techniques that show a willingness to accept and give constructive feedback for the

    purpose of promoting professional excellence.

    • f. Effectively communicates pertinent patient/significant other information during shift-to-shift report according to

    department standards.

    • g. Informs and involves the patient and family prior to procedure or care being performed.
    • h. Seeks direction and clarification of patient care needs and team responsibilities when necessary.
    • i. Follows staffing guidelines. In the interest of patient care (Remove) Responds to changes or needs within the Patient

    Services Team by cooperating with staff rotation, reassignment to other departments with regards to changes in the unit's

    Holiday rotation.

      1. Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and Joint

    Commission National Patient Safety Goals.

    • a. Maintains safety precautions while providing patient care including infection control isolation, patient/bed positioning,

    side rails, skin care, patient/specimen transport, physical environment, fall prevention, seizure precautions, and work

    injury prevention.

    • b. Maintains equipment in good working order, reports equipment failure, and initiates corrective action. Where appropriate adheres to defined computer generated quality monitoring or procedures for validation of equipment functionality.
    • c. Demonstrates recognition of equipment or technological adaptations dependent upon age-specific patient needs.
    • d. Safely handles body fluids using knowledge of appropriate standard precautions.
    • e. Consistently utilizes 2 patient identifiers before performing any diagnostic or therapeutic intervention.
    • f. Cleans equipment according to the germicidal disposable wipe contact time and cleaning procedure.
      1. Actively participates in and supports departmental and organizational quality initiatives and goals.
    • a. Identifies professional goals, which reflect own strengths and weaknesses and seeks assistance for improvement of

    performance.

    • b. Attends and participates in meetings according to department standards.
    • c. Supports and participates in department, hospital and evidenced based care quality initiatives
    • d. Participates as a team member or in workgroups on department, hospital and/or evidence-based care quality initiatives.
      1. Maintains clinical competency according to unit-specific standards.
    • a. Demonstrates competency in maintaining specific technological skills required by job category and/or specialty area.

    Demonstrates recognition of equipment or technological adaptations dependent on age-specific patient needs (See Scope of

    Practice as delineated in departmental standards and/or related Policy and Procedure in the Policy and Procedure Manual).

    • b. Maintains technological competence through practice and ongoing participation in in-services and formalized professional conferences.
    • c. Acquires and maintains proficiency with EKG's, phlebotomy and point of care testing.
      1. Based on staffing needs may be required to work in the Outpatient Lab performing phlebotomist duties.
    • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
    • Other duties as assigned to support the unit, department, entity, and health system organization

    Qualifications:

    • H.S Diploma/ GED Required

    Licenses, Registrations, and Certifications:

    • Certified in B.L.S. for health care providers Required
    • Phlebotomy certification Preferred
