This position will provide case management to pregnant women and children 0-5 years of age.

Examples of Duties

Essential Functions

The employee will explain benefits of Case Management for At-Risk Children and/or Case Management for High Risk Pregnancies. Based on an assessment of identified needs, individualized care plan will be developed and implemented for each client. The nurse will counsel clients/families and provide referrals for other resources as necessary; will demonstrate and implement techniques of nursing care and counsel for the promotion of health and prevention of diseases. Considerable contact with other community agencies and medical homes/physicians will be required in this position; services for clients both in clinical, community, and home settings.

Physical Requirements

This work requires the regular exertion of up to 10 pounds of force, frequent exertion of up to 25 pounds of force and occasional exertion of up to 50 pounds of force; work regularly requires speaking or hearing, using hands to finger, handle or feel, reaching with hands and arms, pushing or pulling and repetitive motions, frequently requires standing, walking, sitting and lifting and occasionally requires stooping, kneeling, crouching or crawling and tasting or smelling; work requires close vision, distance vision, ability to adjust focus, depth perception, color perception, night vision and peripheral vision; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, visual inspection involving small defects and/or small parts, using of measuring devices, operating machines, operating motor vehicles or equipment and observing general surroundings and activities; work regularly requires exposure to fumes or airborne particles, exposure to toxic or caustic chemicals and exposure to blood borne pathogens and may be required to wear specialized personal protective equipment, frequently requires working with volatile, unpredictable patients and occasionally requires working near moving mechanical parts, exposure to outdoor weather conditions, exposure to the risk of electrical shock and wearing a self-contained breathing apparatus; work is generally in a moderately noisy location (e.g. business office, light traffic).

Knowledge, Skills and Abilities

Comprehensive knowledge of County and Department policies and procedures; comprehensive knowledge of the principles, standards, practices and techniques of professional and public health nursing; comprehensive knowledge of treatment, assessment, immunization and related medical services for the type of medically ill patients in area of assignment; thorough knowledge of local and State rules, regulations, procedures and policies pertaining to nursing services; comprehensive knowledge in preparing reports and other types of correspondence; comprehensive knowledge in the use of standard office equipment and associated software (e.g. Patagonia HER software); comprehensive knowledge of and in the use of the equipment required for the position; ability to exercise judgment in appraising situations and making decisions; ability to accurately maintain patient medical records; ability to make arithmetic computations; ability to compute rates, ratios and percentages; ability to solve problems within scope of responsibility; ability to communicate effectively in oral and written form; ability to establish and maintain effective working relationships with patients, physicians and associates.

Minimum Qualifications

Minimum Qualifications

Associates/Technical degree in nursing and moderate experience in professional nursing.

SPECIAL REQUIREMENTS:

Possession of a valid N.C. nursing license upon hire

Valid drivers' license

PREFERRED QUALIFICATIONS:

Experience in home visiting

One year of experience working in a public health environment.
