Green Village Rehabilitation & Wellness is looking for an enthusiastic and compassionate Full-Time Physical Therapist (PT) to join our growing in-house rehabilitation team!

What You'll Do

• Evaluate residents and develop individualized treatment plans
• Deliver evidence-based physical therapy services that maximize functional independence
• Collaborate with occupational therapists, speech therapists, nursing staff, physicians, and the interdisciplinary care team
• Document evaluations, treatments, progress notes, and discharge planning using Net Health and PCC
• Educate residents and families on therapeutic exercises, mobility, safety, and home recommendations
• Help create positive, meaningful outcomes for every resident you serve

Qualifications

• Current or eligible Ohio Physical Therapist license
• New graduates are encouraged to apply
• Skilled nursing or long-term care experience is a plus, but not required
• Excellent communication, clinical reasoning, and teamwork skills
• Experience with Net Health and PCC is preferred
