Full Job Description
Nexus at Belleville is seeking a Physical Therapist Assistant (PTA) to provide high-quality physical therapy services in a skilled nursing setting. This role supports residents in improving mobility, strength, balance, and overall functional independence under the supervision of a licensed Physical Therapist (PT).

Responsibilities
- Implement physical therapy treatment plans developed by the supervising Physical Therapist
- Assist residents with therapeutic exercises, gait training, balance activities, and mobility training
- Provide interventions to improve strength, endurance, coordination, and functional mobility
- Educate residents on safe transfer techniques and mobility strategies
- Monitor patient response to treatment and communicate progress or concerns to the supervising PT
- Assist with restorative and maintenance therapy programs as needed
- Accurately document treatment sessions, progress notes, and resident responses in a timely manner
- Collaborate with an interdisciplinary care team including nursing, rehabilitation staff, and physicians
- Support discharge planning and safe transition of care when appropriate
Qualifications
- Associate’s degree from an accredited Physical Therapist Assistant program
- Active Illinois PTA license (required)
- Experience in skilled nursing or long-term care preferred
- Strong understanding of therapeutic exercise, gait training, and functional mobility
- New graduates encouraged to apply
