Duties

• Educate patients and their families on home exercise programs and strategies for continued recovery.

• Maintain accurate documentation of patient treatments and progress notes.

• Collaborate with interdisciplinary teams in skilled nursing facilities, acute care settings, and outpatient clinics.

• Participate in discharge planning to ensure a smooth transition for patients after therapy.

Experience

• Knowledge of anatomy and medical terminology is essential for understanding treatment protocols.

• Experience in skilled nursing facilities, acute care, geriatrics, orthopedics, or outpatient settings is preferred.

• Familiarity with inpatient care processes and discharge planning will enhance your effectiveness in this role.

• Strong communication skills to interact effectively with patients, families, and healthcare professionals.
