Job Description:
Aventura at West Park is seeking a dedicated and compassionate Full-Time Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will work under the supervision of a licensed Physical Therapist to help residents regain mobility, improve functional independence, and enhance overall quality of life.
Responsibilities:
Provide physical therapy treatments in accordance with established care plans
Assist the Physical Therapist in implementing individualized treatment programs
Monitor and document patient progress and response to treatment
Communicate effectively with interdisciplinary team members
Maintain accurate and timely documentation in compliance with facility and regulatory guidelines
Ensure a safe and supportive environment for all residents
Qualifications:
Associate degree from an accredited Physical Therapist Assistant program
Current Ohio PTA license in good standing
Previous experience in a SNF or LTC setting preferred
Strong communication and interpersonal skills
Ability to work effectively as part of a team
