Schedule: 36–38 hours/week
Setting: Skilled Nursing Facility (SNF)

Position Summary & Responsibilities

  • Assist PT in delivering SNF treatment plans

  • Provide therapy to improve mobility, strength, and function

  • Monitor patient response; report progress to PT

  • Maintain accurate and timely documentation

  • Collaborate with interdisciplinary care team
